Depending on your medical effective date, there are different steps to earning your 2017 Healthy Rewards Rate. Review the information below for more details.

| What is it? | Well-Being Assessment (WBA) – Helps you understand many dimensions of well-being. | Health Screening – Free screening which will include: screening of height, weight, waist circumference, blood pressure and a simple fasting blood draw to screen for cholesterol, glucose, and triglycerides.** | Earn points based on the results of your Health Screening and participate in wellness activities and challenges to earn points. Challenges will include working with a health coach and creating a personal plan, making healthy changes to your diet and addressing stress. | |
| How to do it? | Complete Your WBA on the Healthy Steps Wellness Portal | Visit the Healthy Steps Wellness Portal to schedule an appointment at a local Quest Laboratory Patient Service Center (PSC). If a Quest Laboratory PSC is not available in your area, you must download a personalized Physician Fax form and visit your in-network Primary Care Physician. Please note: The Physician Fax form will be available beginning December 15, 2016 on the Healthy Steps Wellness Portal. |
Your results will be available on the Healthy Steps Wellness Portal within 1 week of completing your screening. Review your results carefully so you can understand what your numbers mean for you. Depending on your screening results, you can earn up to 1,000 additional points. To earn additional points, you have the opportunity to participate in various activities and challenges on the Healthy Steps Wellness Portal, where you can track your activity online OR on your mobile device. (NOTE: All measures must be collected and reported in order to earn up to 1,000 points. In 2017, tobacco-free points will be based on 2016 cotinine results, where available.) |

No action is necessary to earn the Healthy Rewards Rate through 12/31/2017, you will be defaulted to the discounted rate. However, you and your covered spouse/partner can still access the WBA and challenges through the Healthy Steps Wellness portal.
Your health plan is committed to helping you achieve your best health. The Healthy Rewards Rate is available to all employees participating in the wellness program. If you think you may be unable to meet a standard for the Healthy Rewards Rate under this wellness program, you may qualify for an opportunity to earn the Healthy Rewards Rate by different means. Contact us at 877.998.9675 and we will work with you (and, if you wish, with your doctor) to find a wellness option that is right for you in light of your health status to help you earn the Healthy Rewards Rate.
*The Health Screening is FREE when scheduled with a local Quest PSC Laboratory at www.healthystepswellness.com OR when completed by your in-network Primary Care Physician and coded as preventive.
**All information is confidential as protected by federal law and Kindred will not have access to your individual results.
If you and your covered spouse/partner earn 3,500 points by October 10, 2017, you may win prizes. Note that you must complete all the requirements of the Healthy Steps Wellness program, and you must earn the Healthy Rewards rate through the end of 2017 in order to qualify for the prizes.
Your covered spouse/domestic partner must also complete all required activities for you to qualify for the Healthy Rewards Rate.
