How to Earn the Healthy Rewards Rate
To qualify for the reduced/discounted Healthy Rewards Rate, you and your covered spouse/domestic partner must each timely complete wellness activities and challenges that may differ based on your medical plan effective date.

Please note: Covered spouses/domestic partners must also timely complete wellness activities and challenges in order for you to qualify for the Healthy Rewards Rate.
RIGHT TO APPEAL
If you miss a deadline you may have the opportunity to submit an appeal within 60 days of the coverage effective date or the date your rate increases as a result of a missed wellness program deadline. Please see the Benefits Enrollment/Wellness Appeal Request Form cover sheet found on the Resource Center page of the Kindred Benefits Marketplace for more details.
Your specific benefit costs per pay period will be displayed as you’re enrolling in the Kindred Benefits Marketplace. Please note these will be displayed as the Healthy Rewards Rate. If you do not complete the Healthy Rewards requirements, your rate will increase by $75 or $110 per month depending on your coverage tier.
